Health HistoryThe information provided here by the parent, guardian or adult participant is intended to provide UMaine 4-H Center health care personnel with the background needed to provide a safe, healthy and appropriate camp experience. Any changes to this information should be provided to health care personnel upon arrival at camp.
This information will not be used to exclude a participant from participation unless the participant cannot perform program requirements with or without a reasonable accommodation or is determined to be a direct threat to the health or safety of others.
Photo Release AgreementThis form gives the University of Maine System, to which the 4-H centers belong, permission to take photos and videos of our participants to use on our website and other marketing materials.
Release and Assumption of RiskThis form outlines the potential risks of participating in our UMaine 4-H Center programs and acknowledges that you are voluntarily allowing your child to participate.
Research Consent FormThis form gives the University of Maine System, to which the 4-H centers belong, permission to conduct a program evaluation process that includes the analysis of program-wide data, such as participation logs, school attendance records activity logs and staff records, as well as school records for graduation rate, postsecondary enrollment, and community health data. Additionally, we may analyze information from surveys, interviews and focus groups collected from all participants.
