Please note when uploading Health Form H2, a physical and immunizations should also be uploaded to complete the form process. (The Health Form H2, Physical and Immunizations can all be uploaded together or as separate files under the same upload document button link.)

Please scan all forms / documents. No photographs of forms / documents as they are not clearly readable and could cause problems when keeping your camper safe.

Summer Camps follow NYS guidelines for vaccinations and can only accept a medical exemption filled out by a current, licensed medical provider.
