Please Read:

It is important to turn in all forms by the due date to ensure a successful process for all individuals and help campers have a successful safe summer.

List of all Required forms must be completed and uploaded to the document center to complete registration.

1. Physical – This form is provided by your provider. The physical examination must take place within 2 years of your camper’s arrival to camp.


2. Immunizations – This document is provided by your provider and needs to be current.


3. Medication Release Form – (this form is NOT optional) Parent/Gaudian must fill out top of form and sign. If you would like your camper to have any medications at camp, please mark yes and have your provider fill out the bottom half of the form and sign. If marking no at the top of the form a provider does not need to fill out the bottom half of the form.


4. Any incomplete digital forms that did not get completed during the registrations process will be listed above in a red banner under the camp header. Please click on listed links to the forms to complete any incomplete forms.

All documents need to be scanned. No photographs or screen shots will be accepted. All forms need to be uploaded to the document center. No emails of forms will be accepted.

The Camp Registrar will be checking forms for completion. Once forms are approved you will receive an emailed notification of completion. However please note if forms are not complete you will be notified of any missing or incomplete forms during this time. Please be patient for your forms to be verified as there are many accounts to go through.
