This form is required in order to attend the GSOSW Teen Retreat. This information will be shared with the event organizers working directly with youth in order to ensure needs are met. PLEASE INCLUDE DETAILED RESPONSES.

Teen Retreat Program Credit Payment Form
Used to submit Program Credit information to make a payment towards a Teen Retreat registration. NOTE: The full Teen Retreat balance is due upon registration. You must make a reservation, pay the balance with a credit card, THEN complete this form to retroactively apply Program Credits and then your credit card will be refunded once the credits are processed. PROCESSING OF THIS FORM CAN TAKE 3-5 BUSINESS DAYS. Do not use this form to make a Teen Retreat reservation, only to apply Program Credits to your once your reservation has been made.
