Used to submit Program Credit information to make a payment towards a Teen Retreat registration. NOTE: The full Teen Retreat balance is due upon registration. You must make a reservation, pay the balance with a credit card, THEN complete this form to retroactively apply Program Credits and then your credit card will be refunded once the credits are processed. PROCESSING OF THIS FORM CAN TAKE 3-5 BUSINESS DAYS. Do not use this form to make a Teen Retreat reservation, only to apply Program Credits to your once your reservation has been made.
