The Health Care Provider Form, scans of health insurance cards, and Hudson Headwaters Health Network forms may be uploaded Mon - Fri during the summer. Bring your copy to check-in.
The DEC Health Care Provider Form (all camps) and Hudson Headwaters Health Network Form (Pack Forest ONLY) do NOT need to be complete ahead of registering. You may upload completed forms and/or copies of the front and back of your health insurance card ahead of arrival or bring the completed forms to camp check-in.
