This form is required to register.

Camper custody information and emergency contacts during camp session.

Medical History
This form is required to register.

Information about the camper's medical history.

Medical Permissions, Insurance & Provider Information
This form is required to register.

Grants or denies medically related permissions. Provides contact information for physicians and insurance.

Non-Medical Permissions & T-Shirt Size
This form is required to register.

Provide camper t-shirt size. To also give or deny permission for multiple items during the camp session.

Parent/Guardian Completion Acknowledgement
This form is required to register.

Only the parent/legal guardian of the child may complete the forms.

Refunds and Cancellations
This form is required to register.

Acknowledgment of the refund and cancellation dates, deadlines, and procedures.

3. Forms Required to Register for Specific Camp Locations and/or Sessions
Camp DeBruce Notification Acknowledgments
This form is required to register for Camp DeBruce.

Camp DeBruce does not offer swimming and/or boating. Camp DeBruce has vaccination requirements mandated by the Sullivan County Dept. of Health.

Hunter Education
This form is required to register for hunter, bow, and trapper education classes.

If your camper will not be participating in a class, you do not need to fill this out.

Optional: Request for your camper and one friend to be in the same cabin during their session.
Camper's Race/Ethnic Group
Optional: Self-identify camper’s race/ethnic group.
