Release of Liability
I am aware of the nature of the activities offered by Camp Haverim and understand that
my child’s participation in Camp Haverim, extended care, any special program and/or any field trip, including swimming and other activities at Camp Haverim’s facilities, involves a risk of injury. As the parent or legal guardian of the child listed below, I hereby assume all risks and hazards incidental to my child participating in Camp Haverim, extended care, any special program and/or any field trip and hereby release and hold harmless Camp Haverim, its officers, directors, employees, agents, independent contractors and volunteers from and
against any liability for any illness, injury or damage to me, my child or any other family
member occurring during my or my child’s participation in Camp Haverim, extended care, any special program and/or any field trip, including swimming and other activities at Camp Haverim’s facilities.
Publicity Release
I hereby authorize Camp Haverim to use, without any compensation to me or my child, my child’s image, voice or written or artistic work, whether in photographs, recordings, videos or otherwise, to promote Camp Haverim. I acknowledge that the foregoing will be owned by Camp Haverim and that Camp Haverim will have the exclusive right to determine their use.
