Emergency Contact and Medical InformationThis form is required for all overnight and off-site (not at St. Michael Catholic Church or Church of St. Albert) youth ministry events. Parents only need to fill this form out ONCE for their child unless any of the information changes. If emergency contact information or medical information for your child does change, you will need to reopen this form within your account to make the necessary changes.
