Liability & Indemnification: Trip to Convent of the Sacred Heart
LIABILITY RELEASE AND INDEMNIFICATION Visit to Convent of the Sacred Heart - New York City, New York – Tuesday, July 2, 2024 Parent(s)/Guardian(s) hereby give permission for my child, a student at the Summer Experience at Stuart Country Day School of the Sacred Heart of Princeton, NJ to participate in the Field Trip to Convent of the Sacred Heart. They understand that there are risks associated with participation in the program as with all such activities and that there are additional risks to consider including: risks involved with transportation, and outdoor risks. Knowing those risks they sign this Release and Indemnification Agreement as follows: RELEASE/INDEMINIFICATION. In exchange for the opportunity to participate in the program/activity, the participant and the parent/guardians of the participant assume all risks and hereby release, and further agree to indemnify and hold harmless THE STUART COUNTRY DAY SCHOOL OF THE SACRED HEART OF PRINCETON, its present and former parents, affiliates, officers, directors, trustees, agents, employees, and Trip chaperones (“the Releases”) from and against any and all claims for injuries or loss arising out of, directly or indirectly, from participation in the program/activity and/or rendering or failure to render medical treatment, including claims based on any form of negligence on the part of the Releases. If any provision of this Release is void, all remaining provisions shall remain in full force and effect. Please sign and date below thereby agreeing to the above statement.
