This form is required to be filled out by your Band Teacher, Orchestra Director, or Private Teacher.

Designation of Legal Guardian Form
This form is ONLY required if you live further than a day's drive away from Birch Creek's campus in Egg Harbor, WI. Please email this form to an adult who will be responsible for and able to pick up your child in the event of an emergency within a day as described in the Communicable Disease Evacuation Policy.

Health Form
Health information required for all students.

Liability Waivers and Permissions Form
Form required for each student once accepted.

Scholarship Application
Scholarship form information is collected to make a determination on the amount of scholarship dollars assigned to a student. Adjusted gross income (AGI), number of dependents and an audition URL link to a YouTube video/recording are required.

Shuttle Service
This form is required for anyone needing to be picked up from the Green Bay bus station or airport. Due: Not less than 30 days prior to start of session
