CONDITIONS OF ENROLLMENT

PARENTAL CONSENT:
I hereby give consent for my child to participate in all activities of YESHIVA SUMMER PROGRAM (YSP), both on-site (at THE RABBINICAL COLLEGE OF AMERICA and/or other sites chosen by YSP) and off-site, trips, transportation to and from trips, etc., unless I advise you otherwise in writing. I give my consent for my child to go swimming at locations deemed fitting by YSP and am aware that YSP will take campers swimming daily. I affirm, to the best of my knowledge, my child is physically and medically fit to participate in all YSP activities.
Yeshiva Summer Program assumes no responsibilities for pre-existing illnesses, doctor visits, or medical expenses of campers.

PAYMENT AND CANCELLATION:
1. The full tuition cost of Yeshiva Summer Program is $3,900. This includes the registration fee, the doctor's fee, laundry services, and curriculum materials.

2. Yeshiva Summer Program does not offer any scholarships. If a scholarship is received from a third party, the applicant is responsible for paying the tuition and fees in full, and any refunds will only be issued once Yeshiva Summer Program receives the scholarship funds in full. All refunds are processed at the conclusion of camp.
Payment terms are a $300.00 NON-refundable deposit (to be credited towards tuition) to accompany registration. The balance is due by June 15, 2024. Camp fees include all trips, activities, workshops, and laundry. To pay by credit card, complete the credit card authorization form online. There is a surcharge for this service.
Camp fees DO NOT include spending money or canteen money. Camp fees are NON-refundable for late arrivals, cancellations, missed weeks, or early departures. Any cancellations before May 15, 2024, are refundable in full, minus the deposit. If payment in full has not been received by June 15, 2024, we reserve the right to cancel your registration.

3. There is a $25.00 returned check fee. Third-party checks will only be accepted based on Yeshiva Summer Program Administration approval. Anyone residing out of the United States (including Canada) who chooses to pay by check and not by credit card is to pay with checks drawn on U.S. funds in U.S. banks or by international money order. No private foreign checks will be accepted, even if the words U.S. funds are written on the check.

4. Camp fees may be refunded under the following conditions: (a) If the camper is ill prior to going to camp, then a doctor’s certificate attesting to such illness must be sent by June 20, 2024, by registered mail to the Registration Committee, with a request for such refund of camp fees. (b) No refund will be given in the instance where a camper arrives later than the specified date of departure of the camp trip.
(c) No refund will be made in the instance where a camper leaves earlier than the specified date of departure of the camper's camp trip, unless at the Camp Director's behest, due to illness or other causes. In such an event, a refund will be considered. (d) In the event that a camper voluntarily leaves prior to the date of departure of the camper’s camp trip, NO refund will be made. (e) If a camper is removed from the program as a result of non-adherence to all Yeshiva Summer Program rules and regulations, NO refund will be made.

5. Yeshiva Summer Program will not be responsible for damage to or loss of clothing or the personal belongings of any camper resulting from fire, water, theft, laundry loss, etc. Yeshiva Summer Program assumes no responsibility for baggage or packages shipped through any transportation company.

DAMAGE DEPOSIT
All accounts must have a credit card on file. Before the start of the program, a $200 fee will be processed on the credit card on file. Any incidental charges, equipment/property damage fees, and/or extra cleaning fees will be deducted from the paid damage deposit. The remaining balance will be refunded at the conclusion of YSP. If damages are greater than $200, Yeshiva Summer Program will contact the parent/guardian prior to charging the credit card on file. If a student does not incur any additional fees, the entire damage deposit will be refunded, and no additional amount will be put through on the card. No Talmid will gain access to their dorm room without the damage deposit being paid.

MEDICAL INSURANCE
Every camper must have current medical insurance to cover medical bills. Campers that are in need of specific medical attention must submit a credit card number to be kept on file for drug prescriptions and other medical-related expenses. Any medical expenses incurred that are not covered by the camper's medical insurance will be the responsibility of the parent/guardian.

DISMISSAL OF CAMPER:
The Camper, his Parents, Relatives, or Sponsors fully agree to abide by all rules and regulations set by the Camp Director for the health, safety, and welfare of the campers. This includes strict adherence to camp regulations that visiting by parents, friends, or members of the family be limited to the days set aside for visiting days. The Camper, his Parents, Relatives, or Sponsors fully understand and agree that the Camp reserves the right to dismiss, in its sole discretion, any Camper whose condition, conduct, influence, or behavior is deemed unsatisfactory or detrimental to the best interests of the Camp or his fellow campers or who violates camp rules and regulations. In the event of dismissal, tuition will NOT be refunded.

MEDICAL CARE:
In case of emergency, I hereby give permission to the physician as named on this form or, if unavailable, another M.D. selected by the camp director to hospitalize, to secure proper treatment for, and to order injection, anesthesia, or other procedure deemed necessary for my child. Every effort will be made to contact the parent/guardian and emergency contacts first. Should it be necessary for the well-being of the camper to utilize outside medical or dental services, all expenses involved will be paid for by the parent. To the best of my knowledge, my child is in good health, and I will notify the camp if he is exposed to any infectious diseases. I understand that my child may be dismissed during a camp day, due to illness, at the discretion of the camp, and I agree to abide by the director’s decision.
