Privacy Policy:

In completing this application, you are submitting information to be considered for acceptance into Catholic Charities Camp I Am Special or any affiliated program under the supervision or management of Catholic Charities, Camp I Am Special. In addition to this on-line application, additional forms are required to be eligible for consideration. Camp will contact you if additional information is needed and if the applicant has been accepted into a session. Completing this application does not guarantee acceptance into Camp I Am Special.

Catholic Charities Bureau, Inc. and Camp I Am Special of the Diocese of Saint Augustine (hereafter referred to as CCB) respects the privacy of every individual who visits this site. We do not collect personally identifiable information from individuals unless they provide it to us voluntarily and knowingly. If you register to use various features of this site, we use the information you supply for the purpose of providing the services you have requested. We may also contact you from time to time with information that relates to your requests or interests.

CCB collects two types of information: personal voluntary registration information, such as name and email address, and non-personal statistical information, such as site traffic and usage patterns. This information is used primarily to fulfill publication orders, deliver requested information, and to improve the design and structure of the site. It is used solely by CCB and others involved in the operation of this site and will never be sold or given to third parties.

To protect your privacy, we use encryption technologies. In addition, we allow only authorized employees or agents access to personal information. Although we cannot guarantee there will be no unauthorized access to personal information, these measures increase the security and privacy of information traveling to, from and within our web site.

You agree that you will hold harmless CCB and its officers, directors, employees, and volunteers from all claims arising out of or related to your access or use of, or your inability to access or use this site or the information contained in this site or other sites to which it is linked. This includes, but is not limited to, information or materials viewed or downloaded from this website or another website to which it is linked that appear to you or are construed by you to be obscene, offensive, defamatory, or that infringe upon your intellectual property rights. In no event will CCB, its officers, directors, employees, volunteers, or the contributors of information to this site be liable to you or any other third party for any decision made or action taken by you in reliance on such information or for any consequential, special or similar damages, even if advised of the possibility of such damages.

Although we work very hard to provide the user with the most current and accurate information, we cannot and do not warrant that everything you see on this site is up-to-date, error free, or complete. While we may periodically add, modify, or delete any of the content, we don’t make any commitment or assume any obligation or duty to do so. The user should assume that the information is current and up-to-date only as of the date it is posted to this site. Any item with a fee or cost associated may be subject to change at any time, and without prior notification.

Cancellation and Refund Policy:

FOR CAMPERS:

A deposit is non-refundable and holds your spot at Camp I Am Special. If for any reason you decide to withdraw your Camper from a session or need to request a change in session, you must communicate with the office. You will be asked to submit the changes in your Camper’s session in writing, either by mail or email. A refund of fees, minus the nonrefundable deposit, will be considered with a physician’s note and the written request. Such refund will be issued within 30 days at the discretion of the Camp Director.

FOR BUDDIES:

The $50 nonrefundable, once a year, mission commitment fee will hold your spot at Camp I Am Special. If If for any reason you decide to withdraw your Buddy from a session or need to request a change in session, you must communicate with the office. You will be asked to submit the changes in your Buddy’s session in writing, either by mail or email. A refund of the nonrefundable deposit, will be considered with a physician’s note and the written request. Such refund will be issued within 30 days at the discretion of the Catholic Charities.

EARLY DISMISSAL POLICY:

In the event your Camper of Buddy should need to leave Camp for any reason, they must be picked up within 45 minutes of being called.

CANCELLATION POLICY:

Catholic Charities Camp I Am Special may, at our discretion, cancel a scheduled camp session. If your scheduled camp session is cancelled due to unsafe weather conditions, unforeseen circumstances, acts of God, or other circumstances beyond our control, Catholic Charities Camp I Am Special will attempt to reschedule the session. If rescheduling the session is not possible or acceptable to the camper, Catholic Charities Camp I Am Special will review each situation on a case basis, and consideration for a partial or full refund will be made.



HIPAA Notice of Privacy Practices Policy:

Catholic Charities Camp I Am Special is required by law to maintain the privacy of Protected Health Information (“PHI”) in accordance with the Health Insurance Portability and Accountability Act (HIPAA) of 1996. All offices of Catholic Charities that gather information of a medical nature, including Camp I Am Special and the Ministry for Persons with Disabilities, will follow the terms of this notice.

General Release of Liability Policy and Agreement:

For and in consideration of the registered camper being permitted to participate in the Catholic Charities Camp I Am Special and associated programs, the undersigned parent, guardian, or legal representative on behalf of the participant and the participant’s parents, personal representatives, heirs, and next of kin, does hereby release and hold harmless the Diocese of St. Augustine, Bishop Felipe Estevez and any of their employees, agents, or volunteers participating in the Catholic Charities Camp I Am Special programs and their personal representatives and assigns from any loss or damage on the account of any injury to the person or their personal property, or death of the participant while engaged as a participant in the Catholic Charities Camp I Am Special session activities. This release further applies to any claim whatsoever because of first aid, treatment, or service rendered to the participant during the Catholic Charities Camp I Am Special activities.

The undersigned parent, guardian or legal representative expressly agrees that this release, waiver and indemnity agreement is intended to be as broad and inclusive as permitted by the laws of the State of Florida in that if any portion of the Agreement is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

The undersigned parent, guardian or legal representative further acknowledges that he/she is authorized to enter this agreement on behalf of the participant and the participant’s parents, personal representatives, assigns, heirs and next of kin.

General Consent:

The undersigned parent, guardian, or legal representative of the above named, does hereby consent and grant permission for the said camper or participant to attend Catholic Charities Camp I Am Special. I hereby further give my consent for the camper to take part in all camp activities including, but not limited to, athletic competition, crafts, swimming, music, hay ride, etc.

I understand that the camper’s stay at Catholic Charities Camp I Am Special is conditional upon his/her adjustment to daily life and routine at camp. If the camp staff decides that the camper is not adjusting well to camp, the parent will be notified to take the camper home. If a camper’s medical condition, behavior, or personal safety needs exceed the scope of practice provided by Catholic Charities Camp I Am Special, the Camp Director will contact the parent, guardian, or legal representative to take the camper home. Catholic Charities Camp I Am Special will review each situation on a case basis, and consideration for a partial or full refund will be made.

Should a camper’s behavior become combative/aggressive to where he/she could cause harm to others, staff, him or herself, Professional Crisis Management (PCM) procedures will be used. All Group Leaders and many Camp Staff are trained in PCM to ensure the safety and wellbeing of all participants of camp.

Medical Policy:

I hereby give permission to the Camp Director and designated diocesan staff of Catholic Charities Camp I Am Special to medically treat or seek treatment for my camper. In the event I cannot be reached in an emergency, I hereby give permission to the physician selected by the Camp Director or designated diocesan staff of Catholic Charities Camp I Am Special, to hospitalize and secure proper treatment for my camper. I understand I will need to come to the medical facility as soon as possible to attend to my camper. My camper will only be released from the medical facility to my care.

PHOTO RELEASE

Without compensation, I hereby grant permission to the Catholic Diocese of Saint Augustine to use and reproduce photographs and/or video taken of my child. These photographs may be used for news and editorial purposes in publications and other electronic reproductions (websites and video) and/or brochures. In addition, I grant my permission to alter the same photos without restriction and to copyright the same. I hereby release the photographer, the journalists and the publications or media outlets they represent, as well as, the parish/church and/or school involved, the Bishop of the Diocese of St. Augustine, a corporation sole, the Catholic Diocese of Saint Augustine and all of their employees and agents, from all claims and liability relating to said photographs.

I give permission and consent for my child to allow photographs to be taken during Catholic Charities Camp I Am Special activities. I further give permission and consent that any such photographs may be published and used by Catholic Charities Camp I Am Special and the American Camp Association ® and its agents, to illustrate and promote the camp experience, Catholic Charities Camp I Am Special and its camp programs, or the American Camp Association.
