06 - Roommate Request - Summer Programs Only
For 2023, resident campers will be housed in Upton Hastings Hall on the USM Gorham Campus, two campers to a room. We place campers in rooms first by gender, then by age/grade, and consider all special requests. Please use this form to enter a roommate request. If you have no special request, answer "none."

01 - Photography Consent-All Youth Programs
I/We hereby give the University permission to use my/my child’s picture and/or name in the camp yearbook, brochures, newsletters, audio/visual recordings, and other publicity materials that might be produced by the University. I understand that USM does not need my consent to each use of such material. By signing this form you understand that concerts may be livestreamed and available to the public for a limited time. If you decline permission, your child will not perform in concerts.

01 - Youth Ensembles Conditions of Membership
CONDITIONS OF MEMBERSHIP All USM Youth Ensembles members are required to abide by the following conditions of membership. These conditions enable us to maintain a high standard of musical excellence. They encompass the basic considerations and responsibilities that an orchestra or band member must show to their conductor and colleagues throughout the season. Failure to abide by these conditions may result in your non-participation in performances. Attendance: Attendance is mandatory. Excused absences are considered only those in instances of illness, family emergency or conflict with a school musical organization. Other conflicts, no matter how compelling, cannot be permitted. Please evaluate your calendar in accordance with our schedule. You are required to email brackett@maine.edu with any absence as early as possible/ Your absence may have an impact on the conductor’s rehearsal plans. Absences are NOT allowed under any circumstances at dress rehearsals, immersion days or performances. Promptness: All members are expected to arrive 10 minutes before the beginning of their rehearsal, in order to be warmed-up and in tune. Please be aware that traffic into Gorham is very heavy and may increase your commuting time. Assignment of wind, brass and percussion players to the Youth Orchestra and of string bass players to the Wind Ensemble is an honor. Players may not arbitrarily choose to participate in only one of those ensembles because this adversely affects the quality of the ensembles for all the players. Please let us know right away if you are unable to fulfill this obligation. I agree to the CONDITIONS OF MEMBERSHIP as stated above.

This form must be completed at least 60 days prior to the camp starting date, but can be started right away. You will be able to come back in to your account to make changes as needed.

03 - Specific Consents/Authorizations-Summer Programs Only
This form must be completed and maintained by a parent or guardian at least 60 days prior to the camp starting date, but can be started right away. Changes can be made as needed.

04 - T-Shirt Size-summer programs only
Each camper receives one camp T-shirt. Please choose one of the following adult sizes for each camper: XS, S, M, L, XL, 2XL, 3XL

05 - USM Summer Music and Theatre Camps Rules, Regulations and Policies
Please read the Rules, Regulations, and Policies together with your camper. A signature indicates that you have read this together and both understand this and the stated consequences.
