Groton Summer Camp Scholarship Application
Camperships are due by May 30th, 2023 and are available to assist with camp payment based upon need. Applying does not guarantee a campership. All applicants will be notified by mail or email whether a campership is awarded, campership amount and the number of weeks. Please answer the following questions to help determine your eligibility. Primitive Pursuits -Summer Health Form
This is the parent section of the health form. Health forms are considered completed once this form and the Medication/ Immunization records are received completed. The immunization records must be uploaded to your Account. Incomplete health form information may result in the inability for your camper to participate in camp. Both over the counter medications and prescriptions must be signed off on by your healthcare professional.

Groton Summer Camp Health Form
This is the parent section of the health form. Health forms are considered completed once this form and the Medication/ Immunization form are received completed. Incomplete health form information may result in the inability for your camper to participate in camp. Both over the counter medications and prescriptions must be signed off on by your healthcare professional. A PRN order must be obtained by the Camper’s physician, Camper must be able to self-administer the medication, which must come in the original container with only the specific dosages necessary for the duration of the program. Parent/Guardian must sign the PRN.
