It remains an incomplete task until it is audited by staff. You only need to submit one time per winter camp season unless there is a change in medical history.

DECEMBER CAMP REQUIRED: December Camp Travel Form
This form is to provide your travel arrangements to and from camp December 19-21, 2025. Do not fill out this form unless you have registered or are in the process of registering for the December callback camp. If you do not know your travel arrangements yet, skip this form and remember to fill it out once you make your plans.
