This form will be used to help the Medical Director understand the needs and any potential risks of the PRAC members. This is NOT an evaluation of your ability to perform, everyone that registers will be on the field.

OVERNIGHT CAMP REQUIRED FORM: Camp Medical Form 2026
The medical form is a self-assessment of medical history. This form must be completed by the adult participant or by the parent/guardian of a minor. It remains an incomplete task until it is audited by staff. You only need to submit one time per winter camp season unless there is a change in medical history.
