Description
We appreciate your interest in volunteering for MDA’s summer camp program. Quality volunteers are critical to the Association’s goal of providing safe and fun-filled activities for children with neuromuscular disease. Anytime you need to access your account or camp application, you must use the link to the MDA summer camp session sent to you in your original email.

AGE REQUIREMENTS:
Individuals interested must be at least 18 years old in order to apply.

APPLICATION INSTRUCTIONS:
Please be sure the information provided in the application is correct and complete. Any false statement or misrepresentation of the facts called for on this application or any unsatisfactory reference check will be cause for denial of your application or your immediate dismissal from camp at MDA’s sole discretion.

HIPAA TRAINING:
Due to the sensitive nature of the information you will receive on campers, MDA requires all camp volunteers to have the annual Health Information Privacy Protection Act training (HIPAA). If you have not already done so, please view the training using the link below, prior to beginning the application process.

REFERENCES:
Any first-year applicant or those who did not participate in 2019, will be required to provide information for 2 professional references. MDA prefers at least one of the professional contacts be a current or most recent employer or supervisor. The second professional contact must be from academic sources (employers, teachers/professors, guidance counselors), former employers, and other volunteer organizations who have first-hand knowledge of your character, skills and abilities. All references must be at least 18 years of age. Former or current MDA staff and camp leadership may not be used as references. In no circumstance will references from family or extended family members be accepted. MDA reserves the right to contact any reference source for verification purposes in screening volunteer candidates.

Completion of this application does not guarantee you will be offered a volunteer position at MDA Summer Camp. Acceptance is contingent upon evaluation of this application by the MDA Camp Director, MDA medical advisors and other MDA personnel. All information you provide on this form will be used by MDA’s Camp Director to advise appropriate camp staff and medical volunteers only when deemed necessary.

MDA reserves the right to deny admission to camp or dismiss a volunteer whose physical condition, mental condition, behavior, personal conduct or influence on other camp participants is deemed detrimental to the camp community. If this application is not complete, you will not be eligible for camp. A pre-camp PHYSICAL examination is required for all volunteers and must be completed by a physician or medical professional in the twelve months just prior to the camp session or at any time prior to the camp as may be required by MDA.
