Description
UltraCamp functions best on a computer and is not always compatible on tablets or mobile devices. If you do not have access to a computer, or need support in completing the application, please reach out to camp@mdausa.org. Language translation services are also available.

We appreciate your interest in volunteering for MDA Virtual Summer Camp! This program relies on the support and dedication of volunteers, and we’re looking forward to partnering with individuals like you to provide “the best week of the year” virtually for children living with neuromuscular disease.

A COMPLETED ULTRACAMP APPLICATION CONSISTS OF:
  • A recent photo of you
  • An electronically signed Volunteer Application
  • Two professional references (new applicants only)
    Additionally, applicants are required to interview with an MDA staff member and will be screened via criminal background check.

    APPLICATION INSTRUCTIONS
    Please complete the requested information in each section carefully and completely. a response is required in each field. If a question does not apply to you, you must enter N/A to be able to move forward and complete the application. If you are unable to complete the application in a single sitting, be sure to mark the forms as Incomplete, and click Save to be able to return later.

    Completion of this application does not guarantee an offer for a volunteer position at MDA Virtual Summer Camp. Upon review by MDA Staff, you will be contacted with more information and next steps.
