UltraCamp functions best on a computer and is not always compatible on tablets or mobile devices. If you do not have access to a computer, or need support in completing the application, please reach out to camp@mdausa.org. Language translation services are also available.

We appreciate your interest in MDA Summer Camp! It is important to read all the information below, and throughout the application, thoroughly.

All campers must be a member of MDA to participate in MDA Summer Camp. By completing this Camper Application, you/your child will be automatically registered as a member with MDA , if not already a member. There is no fee associated with your MDA membership. MDA Summer Camp opportunities are in high demand. We encourage families to register early, as space is limited. For your child to be considered, MDA must receive a completed application no later than six (6) weeks prior to the start of the camp session.

Once the established application deadline has passed, applications submitted in full are evaluated by MDA Staff and the session’s volunteer Medical Team to determine whether the program is a fit for each child and to confirm the number of volunteers required to support the camp community. Please note that applications are considered on a first-come, first-served basis and the number of campers accepted to a session is often based on the number of volunteers available to support. MDA Staff will extend acceptance notifications in writing once all application reviews are complete and volunteer needs have been met. Submitting an MDA Summer Camp application and/or prior attendance does not guarantee acceptance in any given year.

A COMPLETED ULTRACAMP APPLICATION CONSISTS OF:

  • A recent photo of your child

  • An electronically signed Camper Application and Family & Camper Agreements and MDA Summer Camp Policies form.

  • Accurate and current list of medications your child requires.
    Please be sure any medications your child no longer takes and/or any duplicates are removed from the list.

  • A copy of your child’s insurance card (front and back).

  • Your child’s immunization record.

  • The MDA Camper Physical form, completed and signed by your child’s physician*.
    *It is preferred that the physician completing the form is one most familiar with their neuromuscular disease diagnosis.
