Activity Release Form
This form is required for anyone who is planning to participate in our low ropes, high ropes, climbing and/or mountain biking activities.

Photo Release Form
This form is required to be submitted. If you do not want your child to have their photo taken, please submit this form indicating in writing that you specifically do not consent to photos being taken.

Complete Medical Information
This form must be fully completed and submitted for all week-long camps.

Check In- Check Out Form
This form must be completed for any campers that are attending camp without a parent/guardian/chaperone joining them for the entire duration of their stay.

Emergency Contact Form
This form is required for all campers attending a week-long camp session.

Horseback Riding Release Form
This form is required for anyone who has purchased a horse ride for their week of camp, or if horse rides are included as part of their specific camp session.

If there is a friend you would like to bunk with, please write their name* on this form! *We don't collect church information from our registrants, so if you would like to be with people from a specific church group, please specify their name(s)!

Bring a Friend Discount
If you are bringing a new friend to LVR (who has never attended in past years) fill out this form to receive a 20% discount for each friend you bring!
