Bring - A - Friend Request
Use this form to request your Bring-A-Friend discount. You can receive $20 for a FIRST-TIME camper friend who registers for a Winter Weekend retreat. Your friend must also be fully registered for you to receive the discount. This form is due two weeks before the start of your session. Please understand that the LBC Office will review your request before approving the discount. 1) Register and pay at least the deposit. 2) Complete this form at least 2 weeks before the start of your session (your friend must also be registered) 3) The discount will be added to your account by the LBC Registrar.

Cabin Mate Request - Youth & Teens
This form is for Youth & Teens. The information you share in this form will help us to place you in a cabin for your best possible camp experience. LBC does its best to support your request, but there are circumstances that may prevent your camper from getting into the cabin requested. 99% of the requests are granted.

Cabin Mate Request (Father-Son & Mother-Daughter)
This form is for Parent-Child camps such as Father-Son and Mother-Daughter. At these camps you may be in a cabin with 2 to 3 other families. Use this form to communicate to us your preferences.

Reference Contacts
This form is for applicants to let us know which contacts we should contact for reference information. This form is for Summer Staff & Volunteers.

Medical
The Medical Form is to be completed for Campers, TCL Staff, & Volunteers under 18.

Have ready your medical insurance card and your primary doctor information.
