This form gives Camp a good snapshot of what care and support your camper requires during a weeklong or respite camp session. Counselors will receive this information before your camper arrives to help prepare them to take the best care of your camper. Please be thorough and honest.

Additionally it may be used to determine whether camp can provide services to a camper.

This form is sent home with the camper for their families to read.
