General Forms
Cabin Mate Request - Adult Camps
This form is meant for weekends and camps that have adults staying overnight.
Cabin Mate Request - Youth & Teens
This form is for Youth & Teens. The information you share in this form will help us to place you in a cabin for your best possible camp experience. LBC does its best to support your request, but there are circumstances that may prevent your camper from getting into the cabin requested. 99% of the requests are granted.
Cabin Mate Request (Father-Son & Mother-Daughter)
This form is for Parent-Child camps such as Father-Son and Mother-Daughter. At these camps you may be in a cabin with 2 to 3 other families. Use this form to communicate to us your preferences.
Dress Code Policy
The Dress Code Policy form is to be completed by ALL participants of camp. Campers, Volunteers, TCL, Staff.
Reference Contacts
This form is for applicants to let us know which contacts we should contact for reference information. This form is for Summer Staff & Volunteers.
Health
Allergies and Dietary Restrictions
This form is to be completed for all youth camp participants. It helps the staff provide the best support for specific needs.
Medical
The Medical Form is to be completed for Campers, TCL Staff, & Volunteers under 18.

Have ready your medical insurance card and your primary doctor information.

Registration
Refund Policy
This form is to make sure the person registering the participant understands the LBC Refund Policy.
What NOT to Bring
List of items that are prohibited at camp. Parent will be asked to sign this form.
Serve
Serve Interest Form

Our process for vetting applicants starts here. In order to register as a TCLer, Volunteer or Summer Staff, you must first apply, be accepted, and then you will be emailed a registration link to confirm your commitment to serving at Lakeside in 2027.
 

Use this form if you intend to serve at camp as TCL, Staff, Cabin Leader, Cook, Director, Speaker, First-Aid, Go-fer, Musician, or other special activity.

STEP 1: (This Form) 
  • After you complete this form, you will get an email with the link to fill out the specific application.
  • This follow-up email confirms the completion of this form.
  • In that email you will find the links you need to apply for your area of service.
STEP 2a: Complete the application.
  • The application link is found in the email after you complete this initial intake form.
  • Click on the link to your program that you wish to serve and complete the application.

STEP 2B: Background Check

  • If you are 18+ you must also complete the background check.
  • You will find the link for the BC in the follow-up email after completing this form.
STEP 3: Register
  • If your application is accepted, you will receive an acceptance email for you to then register. The registration link is in the acceptance email after the application is completed & accepted.
  • Use the link in the acceptance email to officially register. This confirms for us that you are coming and that we should be expecting you at camp ready to serve.
  • You will find a unique social media tag (Picture) after you have registered. Use this tag to share with your friends to let them know you are coming to LBC this next year.

  • LBC is unique in that this camp depends heavily on Volunteers, TCL, and Summer Staff to run our programs so as to keep our fees as low as possible.
