Bring - A - Friend Request
Use this form to request your Bring-A-Friend discount. You can request $100 per FIRST TIME friend who registers for the same week of Summer Youth Camp or $20 per FIRST-TIME friend who registers for the same weekend retreat. Please provide the name(s) below. This form is due two weeks before the start of your session. Please understand that the LBC Office will need to review this request in order to approve the discount.

Cabin Mate Request - Youth & Teens
This form is for Youth & Teens. The information you share in this form will help us to place you in a cabin for your best possible camp experience. LBC does its best to support your request, but there are circumstances that may prevent your camper from getting into the cabin requested. 99% of the requests usually get matched.

Dress Code Policy
The Dress Code Policy form is to be completed by ALL participants of camp. Campers, Volunteers, TCL, Staff.

Medical
The Medical Form is to be completed for Campers, TCL Staff, & Volunteers under 18.

Have ready your medical insurance card and your primary doctor information.
